C3 Kids New Family Information Form
Welcome to C3! We love our Children's Ministry and are so excited to welcome your children! In order to provide a secure environment and make sure that you get connected to our community, please fill out the following information about your family and we'll get your kids checked in as soon as you're done!
A few things to note:
* Don't forget to add your spouse and your children in the "Household Members" section.
* When filling out the profile form, please take care when entering names, genders and birthdates of your household, as these are the only fields that will not be editable on your profile without contacting C3 Admin (but she's nice and doesn't mind fixing it!).
* If your children are younger than PreK4, leave the "Grade" field blank. *
